Intensive care unit (ICU) is important in the rehabilitation of critically ill patients. In the past decades, many patients who received aggressive treatment in ICU developed sclerosing cholangitis in multiple centers. Sclerosing cholangitis in critically ill patients (SC-CIP) is a relatively new issue. To investigate the causes, clinical manifestation, treatment, and prognosis of SC-CIP, we searched for published cases in the databases of PubMed, Highwire, and Elsevier from 2001 to 2012. Data were extracted using a standard form and retrospectively analyzed. Twelve eligible studies covering 88 patients, with 64 men and 24 women, were enrolled in this analysis. The mean age was 49.8 years. All of the patients recovered from critical illnesses, such as trauma, infection, burn, and major surgeries. High pressure positive end-expiratory pressure (PEEP, peak level at 12.8 cm H2O) was utilized for all patients, with the average duration of 36.3 d. In addition, vasopressor agents were administered in approximately 60% of SC-CIP. A rapid increase in cholestasis and irregular strictures in the intrahepatic bile ducts was observed in the following months. With an average follow-up period of 17.9 months, poor outcomes were observed in 54 patients, including 34 deaths. In conclusion, ischemic injury of the biliary tree, which may be affected by PEEP and/or vasopressor administration, affects cholangiopathic procedure. As a newly discovered type of secondary sclerosing cholangitis, SC-CIP is a severe progressive complication of patients in ICU and should be carefully monitored by clinicians.

在混凝土类软化材料断裂研究中,裂纹端部损伤区被简化为具有黏聚应力分布的非线性裂纹,该黏 聚力对裂纹扩展有阻抗作用。裂纹体的应力强度因子是断裂力学标志载荷作用与几何构型因素的量化表达指标; 黏聚力形成的阻抗强度因子数值,与黏聚裂纹长度和材料极值拉伸应力存在数量关系。通过双K断裂判据,以 带切口的三点弯曲梁为断裂力学模型,分析了裂纹黏聚阻力对断裂过程的影响规律,计算该弯曲梁结构断裂试 样的最大承担载荷;其结果比较符合实验数据。

Soil nitrogen mineralization (Nmin) is a key process that converts organic N into mineral N that controls soil N availability to plants. However, regional assessments of soil Nmin in cropland and its affecting factors are lacking, especially in relation to variation in elevation. In this study, a 4-week incubation experiment was implemented to measure net soil Nmin rate, gross nitrification (Nit) rate and corresponding soil abiotic properties in five field soils (A–C, maize; D, flue-cured tobacco; and E, vegetables; with elevation decreasing from A to E) from different altitudes in a typical intensive agricultural area in Dali City, Yunnan Province, China. The results showed that soil Nmin rate ranged from 0.10 to 0.17 mg·kg−1·d−1 N, with the highest value observed in field E, followed by fields D, C, B, and A, which indicated that soil Nmin and Nit rates varied between fields, decreasing with elevation. The soil Nit rate ranged from 434.2 to 827.1 µg·kg−1·h−1 N, with the highest value determined in field D, followed by those in fields E, C, B, and A. The rates of soil Nmin and Nit were positively correlated with several key soil parameters, including total soil N, dissolved organic carbon and dissolved inorganic N across all fields, which indicated that soil variables regulated soil Nmin and Nit in cropland fields. In addition, a strong positive relationship was observed between soil Nmin and Nit. These findings provide a greater understanding of the response of soil Nmin among cropland fields related to spatial variation. It is suggested that the soil Nmin from cropland should be considered in the evaluation of the N transformations at the regional scale.

Arthrogryposis multiplex congenita (AMC) is a rare disorder characterized by non-progressive, multiple contractures. In addition to affected extremities, patients may also present microstomia, decreased temporomandibular joint mobility. Although the etiology of AMC is unclear, any factor that decreases fetal movement is responsible for AMC. Thus, accurate diagnosis and classification are crucial to the appropriate treatment of AMC. The development of ultrasound technology has enabled prenatal diagnosis. Very early treatment is favorable, and multidisciplinary treatment is necessary to improve the function of AMC patients. Most patients require surgery to release contracture and reconstruct joints. However, perioperative care is challenging, and difficult airway is the first concern of anesthesiologists. Postoperative pulmonary complications are common and regional anesthesia is recommended for postoperative analgesia. This review on AMC is intended for anesthesiologists. Thus, we discuss the treatment and perioperative management of patients undergoing surgery, as well as the diagnosis and classification of AMC.
